Supported Living Services are provided to persons who are enrolled in the Waiver with a residential need for assistance with safety, or improvement in skills related to activities of daily living, and to build the adaptive skills necessary to function. The goal of this service is to improve the individuals’ independence and fulfill their dreams and aspirations.
The SCH Supported Living Services will take place in a residential community-based setting. A person is eligible for supported living services when they require assistance with the development of social and adaptive skills necessary to successfully reside in the community. It is necessary for the person to have an Individual Support Plan (ISP) and Plan of Care that identifies the need.
Our Supported Living Service includes:
- Eating and food preparation
- Personal hygiene
- Dressing
- Monitoring health and physical conditions
- Assistance with the administration of medication
- Communications
- Interpersonal and social skills
- Household chores
- Mobility
- Financial management
- Problem-solving and decision-making
- Opportunities for social, recreational, and religious activities utilizing community resources
- Appropriate and functioning adaptive equipment
